At our organization, we understand that holistic care encompasses emotional, social, and spiritual support. As a vital member of the interdisciplinary team, you will ensure that patients and families receive the guidance and resources needed to navigate the hospice journey with dignity and peace.
In this role, you will :
- Serve as the psychosocial support expert within an interdisciplinary care team.
- Conduct assessments and develop care plans that address the emotional, psychological, environmental, and cultural needs of patients and families.
- Provide counseling, crisis intervention, and practical resource support aligned with each patient’s personalized care plan.
- Collaborate closely with patients, families, physicians, nurses, chaplains, and other care team members to enhance the emotional and social well-being of the patient-family unit.
- Educate families on hospice services, advance care planning, grief, and coping strategies.
- Assist with long-term planning, community referrals, and resource navigation as needed.
- Monitor and document psychosocial changes in patient or family situations and recommend updates to the care plan as appropriate.
- Uphold ethical standards and exhibit professional judgment in providing end-of-life care.
Qualifications - What You'll Bring :
Master’s Degree in Social Work (MSW) from a CSWE-accredited program.Current licensure as required by the state of employment.A minimum of 1 year of experience as a Social Worker in a healthcare or hospice setting.Understanding of the psychosocial dynamics related to illness, grief, loss, and end-of-life care.Capacity to offer empathetic support while maintaining professional boundaries in emotionally charged situations.Strong communication skills, cultural sensitivity, and respect for diverse family systems.Preferred Qualifications (Not Required) :
3 to 5 years of experience as a Social Worker in hospice or healthcare settings.Expertise in supporting terminally ill patients and their families.Advanced grief counseling or palliative care training.Bilingual skills.We Offer :
