Oncology Patient Navigator

A company is looking for a Supportive Care Guide who provides psychosocial support to patients and caregivers impacted by cancer.

Key Responsibilities

Conduct virtual psychosocial assessments and provide ongoing evidence-based interventions

Develop and implement individual care plans, monitor patient progress, and maintain engagement

Assist patients in navigating the healthcare system and advocate for their needs

Qualifications and Education Requirements

Fluency in both English and Spanish (required)

A bachelor's degree in a healthcare-related field is preferred

Two or more years of experience in a healthcare setting are desired

Experience in oncology care strongly preferred

Certifications such as Certified Patient Navigator or Community Health Worker are strongly preferred
