Position Requirements

• Master of Social Work Degree from a School of Social Work accredited by the Council of Social Work Education.
• Licensed Social Worker (LMSW / LCSW) approved by the State Education Department of the University of the State of New York.
• Two years experience in an acute care hospital setting or related setting preferred.
• NYS SCREEN Certification (obtained within initial 3 months of employment)
• FLU VACCINES REQUIRED - NO EXEMPTIONS

Summary : -

Under the general direction of the Director / Assistant Director / Manager of Pharmacy Services, the Registered Pharmacist provides indirect patient care. The Registered Pharmacist works with nurses, peers, physicians and other members of the healthcare team in the delivery of pharmaceutical services to patients. The Registered Pharmacist reviews the patient's medication orders against the patient's medication profile in accordance with hospital and pharmacy philosophy, policies and procedures and standards.
