Client Access & Outreach Specialist – TSG Health & Wellness
Location : 6636 E. WT Harris Blvd, Charlotte, NC
Schedule : Part-Time (In-Office with Community Events as Needed)
Pay : $15.00 – $18.00 / hour (based on experience)
Mileage Reimbursement : Provided for approved out-of-office community events

Position Summary :
The Client Access & Outreach Specialist serves as the bridge between the community and care.
You’ll manage referrals, verify insurance, assist with Medicaid applications, and ensure every client transitions smoothly into services.
You’ll also represent TSG Health & Wellness in the community — hosting and attending events, promoting programs on social media, and developing relationships with local organizations to expand access and awareness.
This role is ideal for someone who’s organized, personable, and passionate about helping others overcome barriers to care.
Key Responsibilities :
Client Access & Intake Coordination (Primary Focus – 60–70%) :
- Receive and process referrals from MCOs, DSS, courts, hospitals, and partner agencies.
- Conduct intake interviews and collect client information (demographics, insurance, and service details).
- Verify insurance eligibility and assist clients with Medicaid applications, renewals, and pre-authorizations.
- Submit, track, and follow up on authorization approvals to ensure clients can begin services without delay.
- Remove barriers to care by helping clients :
Apply for Medicaid or funding support.
Access affordable or sliding-scale programs when uninsured.Connect to essential community resources such as transportation, food, or housing assistance.Coordinate with the Scheduling Department to ensure new clients are promptly booked for therapy, assessments, or medication management.Maintain compliance with NC DHHS and MCO Access-to-Care timelines and documentation standards.Upload all completed forms, consents, and identification into SimplePractice / EMR prior to service initiation.Data Tracking & Reporting (15%) :
Maintain accurate referral and intake logs to monitor client progress and workflow efficiency.Track metrics such as referral volume, authorization turnaround, and conversion rates.Generate weekly intake and access reports for leadership review.Support audit readiness by ensuring all documentation is complete, current, and compliant.Identify and report trends, barriers, or recurring issues affecting service access.Client Engagement & Support (10–15%) :
Serve as the primary point of contact for new clients, guiding them through onboarding with professionalism and care.Confirm appointments, provide next steps, and assist clients with paperwork, technology, or transportation barriers.Educate clients about TSG Health & Wellness’ integrated care model , connecting them to both behavioral and medical services.Collaborate with the Program Supervisor (QP) and Medical Coordination & Support Specialist (MA) to ensure a seamless transition into care.Community Outreach, Marketing & Engagement (10–15%) :
Represent TSG Health & Wellness at community events, health fairs, and outreach activities.Host and attend events to raise awareness of available services and build local referral partnerships.Develop and maintain relationships with DSS, hospitals, schools, courts, and community organizations to strengthen referral pipelines.Assist with social media outreach — capturing event photos, writing brief captions, and showcasing community involvement.Coordinate local outreach campaigns that promote access to care and brand visibility.Track outreach participation and maintain mileage documentation for reimbursement.Qualifications :
High school diploma or GED required; associate’s or bachelor’s degree preferred.1–2 years of experience in intake coordination, healthcare navigation, or community outreach.Knowledge of Medicaid eligibility, MCO authorizations, and managed care systems preferred.Strong data entry, tracking, and reporting skills (Excel, Google Sheets, or SimplePractice reports).Excellent communication, interpersonal, and organizational abilities.Comfortable engaging the public and representing the organization at events.Compassionate, reliable, and mission-driven — committed to equitable care for all.Why Join TSG Health & Wellness :
