Speech Language Pathologist (SLP) - Flagstaff, AZ

Schedule : Monday-Friday, 8 : 00 AM - 5 : 00 PM | Pay : $75 / hour | No weekends or overtime

Position Summary :

We are seeking a licensed Speech Language Pathologist to evaluate and treat residents / patients with speech, hearing, swallowing, and cognitive challenges. This role involves providing therapy, developing treatment plans, and collaborating with physicians, rehab staff, and nursing teams. Entry-level candidates are welcome to apply.

Requirements :

  • Bachelor's degree in Speech Language Pathology
  • Active Arizona SLP license
  • TB test (2nd Step or QuantiFERON Gold)
  • CPR / BLS Certification

Key Responsibilities :

  • Assess patients and create individualized therapy plans
  • Provide direct therapy and monitor progress
  • Educate residents / patients and families on therapy techniques
  • Maintain accurate documentation and comply with HIPAA
  • Collaborate with interdisciplinary teams

    Resume, Level 1 Fingerprint Clearance, OIG / SAM Check, CPR / BLS Card, TB Test, License / Certification, Hepatitis B & COVID vaccination or declinations, Agency Skills Assessment Packet
