Per Diem UAS Registered Nurse (RN) - The Bronx

About the Opportunity\n\nRockaway Home Care is hiring Per Diem Registered Nurses (RNs) and Outpatient Nurses to support our growing patient population throughout Manhattan, The Bronx, Brooklyn, and Queens. • Conduct in-home or community-based assessments\n
• Perform head-to-toe evaluations including medication and diagnosis review\n
• Complete aide supervisions and provide clinical guidance\n
• Develop and update personalized Plans of Care (POC)\n
• Provide orientation to aides based on the POC\n
• Accurately document findings and maintain compliance with healthcare regulations\n\nRequirements\n
• Current New York State RN license\n
• Ability to travel throughout the 5 boroughs, Nassau, and Suffolk\n
• Medical Professional Liability insurance\n
• Comfortable using Gmail, Microsoft Word / Excel, and UAS-NY (for home care roles)\n\nPreferred Qualifications\n
• Bilingual in Spanish / English\n
• Familiarity with Medicaid, Medicare, or MLTC plans\n
• Experience in outpatient nursing, home care, or community health\n
• Proficiency in the UAS-NY platform\n
