Case Manager / Social Worker
We are looking for a Case Manager / Social Worker to become a vital part of our team at Sunny View Nursing Home , a small 57-bed facility serving both long- and short-term skilled patients.
This integrated role focuses on optimal by bridging clinical case management and psychosocial support. The Case Manager / Social Worker assesses residents' physical, mental, and social needs , develops individualized care plans , coordinates services with the interdisciplinary team and families, and advocates for residents to ensure the highest practical level of physical, mental, and psychosocial well-being and quality of life. This role acts as a central point of contact, identifying barriers to care, and maintaining communication to improve health outcomes and continuity of services.
Key Responsibilities
Care Coordination & Clinical Management
- Assessment & Care Plan Development : Conduct comprehensive physical, mental, and social assessments to understand residents' unique needs, preferences, and abilities. Create and implement individualized care plans in collaboration with residents, families, and the interdisciplinary healthcare team.
- Monitoring & Evaluation : Regularly monitor residents' progress, evaluate the effectiveness of the care plan, and make necessary adjustments to improve outcomes.
- Coordination of Services : Arrange and coordinate services such as medical appointments, therapies, personal care, home visits, and complete Admission Assessments and discharge planning .
- Liaison & Communication : Serve as the primary point of contact , facilitating communication and collaboration among residents, families, healthcare providers, and community agencies.
Financial & Resource Advocacy
Insurance & Authorization : Maintains insurance updates and authorizations. Serves as primary liaison between the facility, residents / families, and third-party payers (Medicare, Medicaid, Managed care, Commercial insurance).Reimbursement Continuity : Ensures that all authorizations, concurrent reviews, and coverage extensions are obtained in a timely manner to support continuity of care and accurate reimbursement .Resource Connection : Maintain and utilize a listing of current community resources that are useful to residents and their families / significant others. Connect residents and families with legal professionals, especially for issues like power of attorney or estate planning.Psychosocial Support & Resident Rights
Psychosocial Needs : Work with residents to identify their psychosocial, mental, and emotional needs, and provide, develop, and / or aid in the access of services to meet those needs.Resident Advocacy : Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological well-being of each resident. Prevent and address resident abuse as mandated by law.Support & Self-Advocacy : Work with residents, families, significant others, and staff to provide support, information, and organization for taking a more proactive role in self-advocacy.Documentation & Quality Assurance
Record-Keeping : Maintain accurate and organized records of assessments, care plans, progress notes, referrals, and other relevant interactions. Understand and meet all government requirements for social service documentation. Document progress in meeting the psycho social needs of residents.Team Participation : Participate in quality assurance interdisciplinary team meetings .Essential Skills and Qualifications
Clinical / Psychosocial Expertise : Strong clinical knowledge to understand and manage various health conditions, as well as a deep understanding of psychosocial, mental, and emotional needs.Experience : Must have experience in working with long-term care residentsInterpersonal Skills : Excellent communication, counseling, and interpersonal skills to effectively interact with diverse individuals and teams, and deal tactfully with personnel, residents, families, visitors, and government agencies.Organizational Skills : Strong organizational and time-management skills to manage multiple cases and coordinate complex services.Problem-Solving & Leadership : The ability to identify and address barriers to care, intervene in crises, and possess leadership ability with a willingness to work harmoniously with all staff.Understanding of Healthcare Systems : Knowledge of the healthcare continuum, third-party payer requirements, and resources available for long-term care
