Essential Duties and Responsibilities

• Provide exceptional psychosocial support to hospice patients and their families.
• Conduct thorough assessments of patients' emotional, social, and psychological needs, and develop comprehensive care plans accordingly.
• Collaborate with the interdisciplinary team to ensure coordinated and holistic care.
• Offer counseling and emotional support to patients and their families, helping them navigate the challenges and emotional complexities of end-of-life care.
• Connect patients and families with community resources, support groups, and other relevant services.
• Advocate for patients' rights and empower them to make informed decisions regarding their care.
• Maintain accurate and confidential documentation of all interventions and assessments.
• Provide bereavement support to families during the grieving process.
