As a key member of Intermountain Home Services, you’ll work directly within healthcare facilities to support seamless patient transitions to home-based care. What You’ll Do As an RN Liaison, you’ll be the vital link between hospital care and home-based healing. Your role will include : Serve as a clinical ambassador for Intermountain Home Services, educating referral sources and promoting appropriate home-based care options Assess patients for eligibility and readiness for home services, coordinating timely transitions from hospital to home Collaborate with physicians, case managers, and interdisciplinary teams to develop and implement patient-centered care plans Support discharge planning by reviewing medical records, identifying care needs, and ensuring accurate intake documentation Communicate and document all care coordination activities clearly and professionally Address barriers to care—financial, logistical, or psychosocial—while advocating for patients and families across the care continuum Ensure timely and accurate completion of orders and documentation to support seamless care transitions Why Join Us as an RN Liaison? To learn about additional Intermountain benefits Click here Minimum Requirements Current Registered Nurse (RN) license in the state of practice Basic Life Support (BLS) certification for healthcare providers Exceptional oral and written communication skills Bachelor of Science in Nursing (BSN) from an accredited institution required within four (4) years of hire, unless already obtained (degree verification required). Existing education agreements will take precedence Ability to work effectively in a collaborative, multidisciplinary environment Self-motivated with strong organizational and critical thinking skills Preferred Qualifications BSN at time of hire from an accredited institution (degree verification required) At least one (1) year of clinical experience in home health or hospice Proficiency in using data to assess needs, measure outcomes, and drive improvement Adaptability and comfort working in a dynamic, feedback-driven environment Strong interpersonal skills with the ability to build and maintain trusted relationships across teams and with external partners Physical Requirements : Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. Need to walk and assist with transporting / ambulating patients and obtaining and distributing supplies and equipment. This includes pushing / pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.) May be expected to stand in a stationary position for an extended period of time. For roles requiring driving : Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
