Qualifications : Required :
- Must have a Master's degree or higher.
- Should hold one of the following licenses / certifications :
- Nurse Practitioner (NP) in Iowa (IA)
- Physician Assistant-Certified (PA-C) in Iowa (IA)
- 2+ years of experience in Nursing Primary Care
- Familiarity with Work Relative Value Units (RVUs).
Preferred :
Experience in fee-for-service or Value-Based Care (VBC) settings.

Summary : The role of the facility-based Advanced Practice Provider (APP) is to provide direct patient care in post-acute care settings.
Essential Duties and Responsibilities :
- Provide patient care adhering to the company's philosophy.
- Conduct Annual Wellness Visits and other clinical assessments as required.
- Perform clinical interventions, evaluations, and treatments.
- Coordinate care with collaborating physicians and other healthcare professionals.
- Maintain communication with families and attend conferences when possible.
- Perform other assigned duties.
Qualifications :
- Must hold a valid nurse practitioner or physician assistant license and be board-certified.
- Bachelor's and Master's degrees in Nursing are required.
- Experience in nursing is necessary, with some preference for Nurse Practitioner / Provider experience.
- Critical thinking skills and knowledge of geriatric care are essential.
- Strong leadership, communication, and problem-solving skills are preferred.
- Punctuality, organizational skills, and teamwork ability are necessary.
