Premier Rehab & Healthcare is seeking a Full Time Wound Care / Treatment Nurse for the facility.
To be part of the Interdisciplinary Team within the facility and weekly rounding with the Wound Physician Group completing assessments and implementing interventions to prevent and heal current wounds and developing the plan of care.
This would be a 40 hour position, times can be somewhat flexible except for day rounding with Wound Physician / NP / PA.
Responsibilities :
1. Conduct comprehensive wound assessment including identifying interventions to be implemented supporting a comprehensive plan of care based on a root cause analysis.
2. Complete documentation weekly support comprehensive wound assessments, update the plan of care and support the nursing staff with obtaining orders for new treatments or interventions with the physician and updating the resident or resident representative with and changes in the condition of the wound.
3. Effective communication with primary physicians, wound physicians, resident and resident representative regarding risks for skin breakdown.
4. Provide education on wound prevention to the resident, resident representative, and staff during orientation, annually and PRN.
Would prefer 2 years experience doing wounds care in a long term care facility.
Wound certification is preferred but not required.
