Social Worker

Essential Job Duties :
- Conduct Social Work assessments to determinethe psychosocial needs preferences and goals of theparticipants.
- Actively participate in IDTmeetings to develop participant care plans based on assessmentfindings.
- Deliver and document social workinterventions as outlined in participants care plans includingarranging necessary resources and services providing counseling andoffering case management.
- Collaborate with theprimary care physician and other members of the care team to ensuresmooth care transitions between settings.
- Initiate coordinate and facilitate care conferencemeetings to enhance care coordination among care team membersparticipants and their support network.
- Provide discharge planning for participants disenrollingfrom the program.
JobRequirements :
- MastersDegree in Social Work (MSW) required.
- Minimumof one (1) year of experience with a frail or elderlypopulation.
- Previous experience coordinatingand facilitating care conference meetings.
- Preferred previous experience assisting individuals withbehavioral health and substance abuse issues.
