Duties :
As a Nurse Practitioner, you will play a crucial role in managing new admissions, conducting medical clearances, and ensuring the medical stability of our clientsYour expertise will help guide our interdisciplinary team in providing comprehensive care that aligns with our treatment philosophyClient Assessment : Assess and treat clients with medical conditions requiring care and intervention, ensuring a holistic approach to their healthDocumentation Excellence : Complete all required documentation promptly, including assessments, treatment plans, progress notes, group notes, and discharge summaries, in accordance with established standardsCollaborative Care : Work closely with other members of the treatment team to ensure cohesive and comprehensive care for our clientsEngagement : Attend and actively engage in mandatory staff meetings and training sessions to stay updated on best practices and facility protocolsCommunication : Maintain strong communication with other clinical team members, outpatient providers, and family members to ensure continuity of careSupervisory Compliance : If required by state law, provide services within a supervisory or collaborative framework, ensuring compliance with Physician Supervision Agreements or Physician Collaboration AgreementsAdditional Duties : Perform additional duties as assigned to support the overall mission of Hoag HealthRequirements :
Minimum of two years of experience as a Family Medicine Nurse Practitioner requiredCalifornia Nurse Practitioner license in good standingGraduate of an accredited Nurse Practitioner programPediatric experience is highly preferredStrong communication skills for patient education and interdisciplinary collaboration.Compassionate approach towards patient care with an emphasis on empathy and support.Epic knowledge is preferredContact : Steven Yi
