Hoag Health, Orange County's top-ranked health system is seeking an experienced and dedicated Primary Care Nurse Practitioner to join our growing team! The ideal candidate will provide exceptional care to patients of all ages at our Aliso Viejo, CA location. This role requires a strong commitment to patient advocacy and the ability to work collaboratively within a multidisciplinary team to ensure the best outcomes for our patients. Details :

Full time position. Monday-Friday
Competitive compensation : Base guaranteed salary

Outpatient based role. No weekends or after hour requirements
Experience and passionate primary care physicians to provide support and mentorship
Seasoned and tenured support staff
EMR : Epic

Duties :

As a Nurse Practitioner, you will play a crucial role in managing new admissions, conducting medical clearances, and ensuring the medical stability of our clients

Your expertise will help guide our interdisciplinary team in providing comprehensive care that aligns with our treatment philosophy

Client Assessment : Assess and treat clients with medical conditions requiring care and intervention, ensuring a holistic approach to their health

Documentation Excellence : Complete all required documentation promptly, including assessments, treatment plans, progress notes, group notes, and discharge summaries, in accordance with established standards

Collaborative Care : Work closely with other members of the treatment team to ensure cohesive and comprehensive care for our clients

Engagement : Attend and actively engage in mandatory staff meetings and training sessions to stay updated on best practices and facility protocols

Communication : Maintain strong communication with other clinical team members, outpatient providers, and family members to ensure continuity of care

Supervisory Compliance : If required by state law, provide services within a supervisory or collaborative framework, ensuring compliance with Physician Supervision Agreements or Physician Collaboration Agreements

Additional Duties : Perform additional duties as assigned to support the overall mission of Hoag Health

Requirements :

Minimum of two years of experience as a Family Medicine Nurse Practitioner required

California Nurse Practitioner license in good standing

Graduate of an accredited Nurse Practitioner program

Pediatric experience is highly preferred

Strong communication skills for patient education and interdisciplinary collaboration.

Compassionate approach towards patient care with an emphasis on empathy and support.

Epic knowledge is preferred
