Job Title : Physical Therapist - Home Health

Job Specialty : Physical Therapist

Job Duration : 13 weeks

Shift : Days, 40 hours per week, Monday-Friday, with one weekend a month rotation

Guaranteed Hours : 40

Experience : Minimum of 1 year of Home Health experience

License : CA PT License

Certifications : N / A

Must-Have : Training includes 3 full days, one in-house and two in the field

Job Description :

• Prioritize start of care visits.
• Supervise Physical Therapy Assistants.
• Mandatory next-day reporting of start of care visits.
• Submit clinical notes and other documentation within 48 hours of the visit.
• Maintain timely communication with supervisor, intake, and office.
• Be generally responsive during normal business hours.
• Territory coverage includes required travel within specified counties with potential to expand.
• Utilize the Kinnser documentation system.
• Equipment provided includes an iPad and communication device.
• Mileage reimbursement provided for travel between patient visits.
• Company car available as an option with adjustments.
• Productivity requirement of 30 points per week with specific visit and point values.
