Description :
The Home Hospital Registered Nurse provides direct patient care to inpatients at the acute level within the comfort of the patient’s own home environment.
Each patient has an individualized plan of care that determines the tasks and duties for home visits.
The Home Hospital RN is responsible for providing the essential elements of the inpatient care delivery model within the patient’s defined living space using leveraged technology and remote clinical care coordination.
The Home Hospital RN provides quality care to patients through the implementation of the nursing process applied through a team nursing model.
The Home Hospital RN will access and document the care of patients and will work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient’s home while maintaining high-quality and safety standards.
Experience managing acute care or medical-surgical adult and / or geriatric patient populations is preferred.
Telephone triage experience is preferred.
Home health experience is preferred.
Rotating Day / Evening / Weekend and Holidays shifts are required.
Candidates should be willing and able to float within their scope as needed.
