Job Title : Case Manager - RN

Must-Have :

• Graduate from an accredited school of nursing; BSN preferred
• Two years of healthcare experience as a Registered Nurse
• Epic experience required
• Acute hospital experience required

Description :

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.

The overall goal of the position is to enhance the quality of patient management and satisfaction.

This role promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning.

The Case Manager must be a highly organized professional with great attention to detail.

Adaptability to frequent changes is essential.

The individual must comply with regulatory and departmental guidelines and policies.

Weekends are required.

No call responsibilities are included.

Candidates must be able to work effectively in a fast-paced environment and ensure a smooth transition of care for patients.

Experience with admission criteria, care coordination, and discharge planning is necessary.

Knowledge of concurrent reviews, appeals and denials, and utilization of InterQual and Milliman guidelines is essential.

This role involves working with a diverse patient population, including neonates, infants, adults, and geriatrics.

Ability to navigate various clinical settings and collaborate with interdisciplinary teams is critical for success.
