Required Procedures : TRUS Biopsies of prostate, cystoscopy, cystoscopy and dilation

EMR : EPIC

Support Staff : 2 RN's, 1 LPN

Reason for Coverage : No permanent provider on staff

SUBMISSION REQUIREMENTS :

Board Certified - Preferred (Will accept Board Eligible providers if they are within 5 years from residency)

North Dakota license - Preferred (IMLC is okay)
