Job Title : Social Worker (MSW)

Location : Napa, California

Salary Range : $85,904 – $133,348 per year ($41 - $64) / Hr.

Position Summary :

A leading healthcare provider in Napa, California is seeking a compassionate and experienced Social Worker (MSW) to support inpatient care. Under minimal supervision and guided by the Case Management Director, the Social Worker / Patient Counselor plays a key role in meeting patients' bio-psychosocial needs and facilitating access to medical and community resources.

This position involves providing crisis intervention, short-term counseling, and coordinated discharge planning to ensure smooth transitions across the continuum of care.

Key Responsibilities :

• Conduct comprehensive psychosocial assessments for patients.
• Deliver short-term counseling and crisis intervention services to patients and families.
• Develop and implement effective discharge plans tailored to each patient's needs.
• Collaborate with healthcare professionals and community agencies to support patient care.
• Advocate for patient access to services and ensure continuity of care post-discharge.
• Maintain accurate and timely documentation in compliance with regulatory standards.
