Job Summary & Responsibilities :

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete therapeutic care process for an assigned group of patients in coordination with physicians and the care team. This role includes patient assessments, developing and implementing individualized care plans focused on goal achievement, patient engagement, and positive patient outcomes. The PT delivers or directs most physical therapy interventions throughout the continuum of care.

Key Responsibilities Include :

• Providing evaluations and treatment interventions within legal and professional practice scope.
• Assessing therapeutic, rehabilitative, and functional status to develop comprehensive care plans.
• Delivering skilled therapeutic care, including home care, consistent with physician's plan.
• Instructing patients, families, and caregivers in therapies, exercises, and use of therapeutic devices outside therapy sessions.
• Prioritizing caseloads, determining staffing needs, and coordinating assignments and handoffs .
