Oregon Licensed Care Manager (RN)

A company is looking for a Care Manager (RN) - Complex Care.

Key Responsibilities

Develop and facilitate complex care management activities and personalized care plans for members with physical needs

Coordinate care between members, families, and providers to ensure appropriate person-centered services

Monitor member progress and revise care plans as necessary, while providing education and resource support

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in nursing or care management

RN - Registered Nurse - State Licensure and / or Compact State Licensure required
