Position Overview
We are seeking a compassionate, skilled, and experienced Hospice RN Supervisor to join our dedicated hospice care team in Milledgeville. This full-time leadership role is essential to ensuring that patients receive the highest standard of end-of-life care in accordance with our mission of dignity, comfort, and compassion.
The RN Supervisor will not only deliver direct patient care but also serve as a clinical leader , managing day-to-day hospice field operations, supporting and supervising staff, and coordinating care with interdisciplinary teams. Ideal candidates are empathetic clinicians with hospice experience, a strong sense of initiative, and a passion for mentoring others in a supportive, patient-centered environment.
Salary & Benefits
- Annual Salary Range : $80,000 – $90,000
- On-Call Pay Structure :
- $150 per weekday on-call (Monday – Thursday)
- $450 for weekend on-call (Friday – Sunday)

Clinical Supervision & Leadership
Oversee and support day-to-day clinical operations across the hospice teamSupervise LPNs and CNAs, ensuring high standards of clinical care and professionalismConduct supervisory field visits to evaluate care delivery and staff performanceMentor and guide staff nurses in clinical decision-making, patient communication, and documentationPatient Care & Case Management
Perform skilled nursing visits, including start-of-care (admissions), routine, and PRN visitsProvide comprehensive head-to-toe assessments and pain / symptom managementDevelop, update, and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG)Educate patients and caregivers on disease progression, medications, and hospice philosophyRespond to changes in patient condition with clinical interventions and communication with the medical directorAdmissions & Acute Visits
Lead the admission process by completing initial assessments, obtaining consents, and setting care expectationsCoordinate DME delivery, medication orders, and first visits by all disciplinesBe available for acute needs and symptom management as they ariseInterdisciplinary Coordination
Attend and actively participate in weekly IDG meetings to review and coordinate ongoing care plansCollaborate with physicians, social workers, chaplains, and therapists to ensure holistic careAct as a liaison between families, team members, and the organization to ensure clear communicationCompliance & Quality Assurance
Ensure documentation is accurate, timely, and compliant with Medicare and agency standardsParticipate in QAPI (Quality Assurance and Performance Improvement) activitiesMonitor clinical metrics, patient outcomes, and staff adherence to protocolsImplement corrective actions or performance improvement plans when neededDay-to-Day Duties
A Typical Day Might Include
Morning clinical huddle or coordination callReviewing overnight reports or urgent needsPrioritizing and assigning patient visits to LPNs / CNAsCompleting a supervisory field visit with a CNA, followed by documentation and coachingAdmitting a new patient : reviewing the referral, visiting the patient, gathering consents, performing a full clinical assessment, and initiating the care planAttending an IDG meeting to update the team on case progress and collaborate on care strategiesHandling urgent calls from families or field staff to guide symptom management or emergency careEnsuring orders are signed, supplies are delivered, and patient records are updated before day's endBeing on-call after hours or weekends as scheduleWe are an equal opportunity employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status, or any other characteristic protected by law.
