Position : Patient Navigator I
Department : PDD
Reports To : Program Operations Supervisor
POSITION SUMMARY :
Navigators will deliver services to individuals that are being incarcerated in Lancaster at the Sheriff Department in Lancaster as part of the Alternatives to Incarceration Program.
Operating within the justice involved demographic, key responsibilities including but not limited to providing transportation, advocating for patients, and coordinating essential services.
The overarching goal is to contribute to the promotion resiliency, and self-sufficiency among individuals involved in Alternatives to Incarceration Program, fostering a supportive environment that facilitates positive outcomes and personal empowerment.
Salary Range
$24.50 - $27 / Hour DOE
Shifts will include overnights and weekends as applicable.
Requirements :
- High School diploma, or equivalent, is required.
- Bilingual (English / Spanish) a plus
- CA Driver’s License.

SUMMARY OF RESPONSIBILITIES :
Position is responsible for completing preliminary screening to help determine level of care and services to support recovery efforts, as well to help individual access needed health and social support services.
Position conducts on-site and / or field- based outreach, engagement, re-engagement and navigation services and scheduling.
SPECIFIC DUTIES :MINIMUM QUALIFICATIONS :
KNOWLEDGE AND SKILLS :
- Passion and desire to work justice involved demographic.
- Demonstrate a non-judgmental attitude and positive regard for justice involved demographic.
- Experience providing field-based services.
- Strong people skills and ability to work with people of various ages, racial, ethnic, cultural, educational and socio-economic backgrounds.
- Organized, detail oriented and creative problem-solving.
- Proficient in computer skills and use of Microsoft products such as Outlook, Word and Excel.
- Must have valid CA driver’s license.
