Registered Nurse for ADHC
Overview :
Central ADHC is a busy and happy CBAS Adult Day-Health Center located on Beverly Blvd near Koreatown / Downtown LA. Providing nurse-led care and monitoring of health conditions and professional Social Work counselling and advice to Senior members of the community coping with social or family loss, loneliness, dietary needs and monitoring of health conditions and / or disabilities that impact the quality of daily life, we are the place to come to! We ensure our Participants feel connected, socially engaged, alert and active, superbly well fed, and health issues monitored – all providing a best outcome and uplift to the day.
Central ADHC provides Transportation from and to the Home, daily vitals, nutritious Breakfast, Snack and tasty Lunch, a wide range of Activities & Movement Therapies, advice on Social Benefits & Application, Nutrition & Diet, and Wellness / Mental Health Counselling and Advice . Responsibilities :
- Assess each participant to determine needs for personal care and assistance in activities of daily living such as bathing, grooming, toileting and eating. Include these needs in the plan of care and supervise the provision of these services. All participants must be reassessed at least every six months.
- Request health information from PCP as background information.
- Specify short-term nursing goals in each individualized plan of care.
- Provide active role as member of the multi-disciplinary team to assess the nursing needs of the participant and recommend plan of care.
- Develop and implement the nursing component in the plan of care for each participant capable of benefitting from a continence training\program.
- Provide health education and counseling to the participant and family when indicated by the plan of care.
- Provide assistance to the participant in obtaining medical services from providers other than adult day health staff.
- Provide observation and monitoring of health status.
- The nurse will contact 911 and request an ambulance or paramedic unit depending on the condition of the participant.
- Document incidents for OCRM and provide intervention as appropriate.
- HIPAA Access : Minimum necessary to document data, access and carry out responsibilities of position.
Qualifications :
Graduation from an accredited nursing program.Current valid License as a Registered Nurse through the California Board of Registered Nursing.Experience in and willingness to be part of multi-disciplinary team.Current Valid BLS CertificationExperience with physically or mentally impaired adults and / or geriatric population.Preferred Skill :
