Job Description :

• As Hospice LPN you will be responsible for helping to manage and coordinate the care of patients in their homes, skilled nursing facilities, or assisted living environments.
• You will work closely with the RN Case Managers to assess patient needs and provide direct nursing care to ensure the comfort and dignity of patients at the end of life.
• This position requires a nurse with clinical expertise, compassion, and a commitment to providing high-quality hospice care.
