Job descriptionProvide comprehensive geriatric care including chronic disease management, frailty assessments, polypharmacy review, and advance care planning Care for adults 65+ with multi-morbid chronic conditions Primarily based in one outpatient location with occasional rotation to nearby clinics Monday–Friday, Days 1 Saturday / month (with a weekday off) 2 evening shifts / month Base Salary + RVU-based incentive plan Extensive benefit package, including health, dental, vision, retirement match, CME, PTO, and malpractice coverage MD / DO, Board Certified / Board Eligible in Internal Medicine Geriatrics fellowship preferred but not required NY State license (IMLC accepted) ACLS / BLS Experience with geriatric populations preferred
