Open to entry level or candidates with no prior home health experience Title :

• Physical Therapist, PT Schedule :  Per Diem (6 visits per week) Locations Available :    Area #45 covering the cities of Agoura Hills, Calabasas, Moorpark, Newbury Park, Oak Park, Simi Valley, Thousand Oaks and Westlake Village and secondary Area #42 covering the cities of Canoga Park, Chatsworth, Encino, Lake Balboa, Reseda, Tarzana, West Hills, Winnetka, Woodland Hills Paid Training : Must be able to train for 5 days Mon-Friday 9am-5 : 30pm in Woodland Hills Position Summary : Initially, a PT will evaluate mobility, strength and current level of function, while setting goals to improve balance, walking, transfers, as well as patient education.
• Unlike a rehabilitation setting or even an outpatient clinic, unique emphasis is placed on safe mobility in and around the patient’s residence.
• PT’s will also make recommendations on equipment and provide train on proper use of equipment.
