Position Overview

We are seeking a Registered Nurse (RN) - Hospice to join our dedicated team. This role involves planning, organizing, and delivering hospice care with a strong emphasis on community health education. As a hospice RN, you will coordinate care for patients and their families, ensuring comfort, dignity, and quality of life.

Key Responsibilities

Patient Care

• Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice care needs.
• Develop, implement, and adjust individualized care plans based on patient conditions and evolving needs.
• Administer medications, treatments, and palliative care interventions as prescribed.
• Educate patients and families on symptom management, medication use, and self-care techniques.
• Provide emotional and psychological support to patients and their loved ones.
• Assist with activities of daily living to enhance patient comfort and independence.
• Serve as a Case Manager, coordinating care for assigned patients.
