Clinical Magnet (part of ICON Medical) is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join a respected healthcare organization serving the Lakewood, Colorado community.

This is a FULL-TIME, DIRECT HIRE opportunity with excellent benefits and competitive pay.

Responsibilities of the LPN :

• Provide high-quality nursing care and support to patients in their homes.
• Administer prescribed medications and treatments in accordance with nursing standards.
• Educate patients and families on medications, treatment instructions, and care plans.
• Document clinical notes, treatments, and progress in the EMR system.
• Monitor patient conditions and report changes to physicians or RNs.
• Collaborate with the care team to ensure patient-centered outcomes.
