Family Practice / Primary Care Physician Assistant

Join a large physician-owned multispecialty group as a Nurse Practitioner

Provide care for homebound patients and local skilled nursing facility (SNF) patients

Deliver proactive, efficient, and cost-effective managed care

Perform house calls and facilitate transitions from hospital / SNF to home

Reconcile medications, educate patients / caretakers, diagnose conditions, and order testing

Communicate with visiting nurse agencies, PCPs, specialists; coordinate care and referrals

Collaborate with other NPs and facility medical directors in SNF settings

Part-time schedule (32 hours per week) with competitive salary and benefits (health, malpractice, 401K, CME allowance, PTO)

Must be licensed in MA (RI license a plus), DEA certificate, and MA controlled substance registration

Seeking candidates with maturity, organizational skills, independence, flexibility, and problem-solving ability

Portuguese fluency is a plus, but not required
