Job descriptionProficient computer skills to complete tasks as required. Must be organized, able to effectively prioritize tasks and exhibit appropriate time management skills. Demonstrates knowledge of and applies evidence-based practice. Collects comprehensive data pertinent to the patient’s health or the situation. Identifies and reports changes in patients’ status to Registered Nurse for evaluation. Documents nursing activities accurately and in a timely manner. Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment under the direction of the Registered Nurse. Administers treatments and other direct care; prepares and administers medications as prescribed; observes and evaluates patients’ responses to medications and treatments; and identifies and promptly communicates adverse reactions to the Registered Nurse. Collects specimens as ordered. Interacts with and contributes to the professional development of peers, colleagues, and students. Collaborates with patients, family, and others in the conduct of nursing practice. LPN is a patient advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction. Attains knowledge and competency that reflects current nursing practice. Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules, and regulations, during nursing practice. Exhibits High Reliability Organization (HRO) tools and tones. Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable. Receive phone calls and messages, return calls to patients and / or family members in a timely manner. Triage patients on phone and walk-ins to department for any acute issues that need addressed in between office visits. Obtaining advice and orders from Providers related to patient issues. Relaying messages from Providers to patients Processing prescriptive medications as ordered by a Provider Form completion for FMLA, Disability, prescription assistance, etc. Referrals to tertiary or quaternary centers as requested by Provider or patient Coumadin Management Any miscellaneous requests by Director, or Provider Attends required meetings and participates in committees as requested. Participates in professional development activities and maintains professional affiliations. Performs department specific work as required. Complies with all regulatory policies and procedures. Primary duty is physician support with documentation in the EHR in a non-intrusive manner to ensure timely dictation and billing. Gather and chart information pertaining to the patient health history, physical exam, diagnostic findings, lab and test results, consultations with other providers as part of care coordination, and patient instruction / education with attention to detail. Directly observes patient encounters and documents the events and decision-making that result in accurate and appropriate medical charting. Assists in the processing of electronic physician orders in the EHR for radiology, lab tests, medications, and all future chemotherapy appointments ensuring accuracy to maintain safety. Proposes all orders to the physician for signature. Able to use critical thinking skills in patient care, order entry, and documentation. Compliant with all policies related to scribing. Able to keyboard proficiently. Attends required meetings and participates in committees as requested. Participates in professional development activities and maintains professional affiliations. Performs department specific work as required such as updating problem lists and chart preparation. Complies with all regulatory policies and procedures. INTAKE / ROOMING Performs selected clinical and clerical duties. Prepares patients for examination. Takes patient histories and vital signs, documenting as trained in the EMR. Completes Distress Screening on all new oncology patients. Prepares exam rooms. Prepares patients and assists physician during procedures. Provides patient instruction / education with attention to detail. Able to keyboard proficiently. Able to use critical thinking skills in patient care, order entry, and documentation. Able to keyboard proficiently. Attends required meetings and participates in committee work as requested. Participates in professional development activities and maintains professional affiliations. Performs department specific work as required such as updating problem lists and chart preparation. Complies with all regulatory policies and procedures. Typically works 8 hours per day, Monday through Friday. May require paid overtime before or after normal working hours. Subject to frequent interruptions during performance of duties. Expected to adhere to office policy and procedures. Meal and break times are coordinated with other staff members to maintain coverage Shifts are scheduled or changed due to staff needs of the unit / department Cross training dependent upon departmental needs.

Position Summary
The primary purposes of the Licensed Practice Nurse (LPN) are to provide direct patient care, patient and family teaching, and work cooperatively within the interdisciplinary health care team in maintaining standards of nursing care under the direction of the Registered Nurse (RN) and within the framework of the Ohio Nurse Practice Act.
Qualifications
- Graduate of an accredited school of nursing.
- Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
- Maintain current, active BLS certification through the American Heart Association.
- IV Certification preferred.
