We are seeking compassionate and empathetic individuals to provide case management services to individuals with disabilities on CADI waivers, Traumatic Brain Injury Waivers, and Elderly Waivers. The successful candidate will have strong listening, organization, and documentation skills. Case Managers are responsible for coordinating services for individuals in order to meet their needs and work alongside people in maintaining as much independence as possible in their communities.
Day to day activities involve phone and email communication with clients, other service providers, and family and community members. Case managers have the opportunity to conduct in-person visits with individuals on their case load twice yearly (these visits temporarily being done virtually when possible due to COVID). Timely documentation of communications and meetings is an essential function of this position. Training is provided to familiarize new case managers with county, state, and federal resources to ensure that client's needs are met and they are able to utilize all available resources to ensure their independence.
