Position Summary / Objective
The St. Vincent de Paul Shelter Case Manager promotes a culture of safety, hospitality, service, honor, and respect among our homeless guest, our staff, and volunteers, for the purpose of furthering the vision and mission of the St.
Vincent de Paul Society.
In collaboration with our community Partners, both on and off-site, the St. Vincent de Paul Shelter Case Manager provides intensive housing-focused services to Gateway residents, frequently meeting and engaging with them in furtherance of obtaining appropriate and affordable housing or program in the community and completing documentation reflecting of this work.
This individual aids guests in creating a dynamic case plan leading to obtaining and community resources. The Case Manager works as a member of an interdisciplinary team to assist residents in achieving the highest goals possible.
Ideally this individual should have a background in Chemical Dependency and mental health Case Management and Counseling, but this is not a must, we can train.
This position is responsible for assisting the Rehousing Manager in monitoring the performance of any Case Manager.
Participate with the Rehousing Manager and Care Teams in the formulation of case management policies and procedures, goal setting, planning, and evaluation processes.
- Oversee daily living activities in the program environment
- Provide support services to Gateway residents
- Building and maintaining a working knowledge of community resources and he issues integral to homelessness, mental illness, mental health treatment modalities, and addiction treatment
Skills and Requirements :
- Experience working with low-income, unemployed, or homeless adults
- Experience with and working knowledge of substance abuse and / or mental health issues, and related community resources.
- Computer / Microsoft Office software
- Good Verbal / Written Communication skills
- Furnish a reliable, personal means of transpiration in order to accomplish work objectives
- Self-motivated and the ability to work independently
- CDC or MHY, Bachelor Level degree preferred, but not required
