Mobile Health Community Paramedic

Position Responsibilities

Provides skilled, professional, and compassionate mobile integrated healthcare to enrolled patients.

Conducts post-hospital release follow-up care including, but not limited to home safety / living condition evaluations, medication reconciliation, wound care, vital signs, and medication administration.

Able to deploy or trouble shoot patient monitoring equipment in the home.

Transport, set up, troubleshoot, maintain, recover / decon and educate patients on the required in-home Tech Packs for the patients.

Observes, records, and reports to physician / nurse patient’s conditions and reactions to drugs, treatments, and significant incidents.

Examines, screens, treats and coordinates health services for patients which may entail point of care laboratory testing based on paramedic’s education, training and competency

Conducts patient education, includes diabetes prevention / treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessments, injury evaluation, geriatric frailty visits, and nutrition.

Functions as nurse / physician / social worker and hospital extender through advanced protocol utilization

Coordinates appointments, additional home health visits and follow up with providers and facilities as appropriate / ordered

Comprehensive knowledge of principles and processes for providing customer services, including setting and meeting quality standards for services and evaluation of customer satisfaction.

Ability to handle a variety of customer service issues with tact and diplomacy in a confidential manner

Ability to communicate ideas effectively, including the preparation of reports and logs. Ability to listen and understand directions, information and ideas presented verbally and in writing

Attends training sessions for new and continuing education, as required.

Perform other duties as assigned depending on the community need and / or home situation.

Performs all job duties and responsibilities as described for Paramedic including but not limited to providing BLS or ALS to patients in an emergent first responder capacity in collaboration with local EMS agencies.

Performs related tasks as required.

Ability to navigate with appropriate sensitivity to the patients and families' diverse cultural, religious and ethnic backgrounds.

Physical Demands

Air conditioned office environment and patient homes.

Occasionally : Confined areas, extreme hot and cold, wet and / or humid conditions,noise, vibration, mechanical and electrical equipment, moving objects, high places,fumes / odors / mists, dirt and dust, gasses, toxic conditions, human excrement, blood, urine, mucous, tissue.

Frequently works alone, with and around others, face to face and verbal contact, inside and outside temperature changes.

Constantly shift work, extended day

Sit for extended periods of time

Walk, stand, bend, squat, twist and reach

Extended keyboarding

Occasionally smelling, lifting up to 125 lbs, pushing, pulling, typing (20 wpm), climbing,balancing, carrying no more than 70 pounds, kneeling, stooping, bending, leaning,upper and lower body flexibility, running distance, driving ambulance, car or truck,multiple physical activities performed at the same time (driving, talking and seeing).

Frequently hearing / listening, clear speech, touching, walking inside and outside,

Education, Experience, and Special Requirements

Minimum 5 years experience at the Paramedic level, in a strong 911 system

Current EMT-P license, CPR, PALS, PHTLS and ACLS cards

Strong desire to continually improve your skills and knowledge

Critical Care Transport, Community Paramedic Certifications preferred

Ability to speak / navigate multiple languages, Spanish, Vietnamese preferred
