The Occupational Therapist is responsible for evaluating patients in their homes and developing individualized therapy plans that help restore daily living skills, independence, safety, and functional mobility.
You will work closely with patients recovering from illness, injury, surgery, or chronic conditions to improve their ability to perform everyday activities.
Conduct comprehensive in-home occupational therapy evaluations
Assess patient ability to perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), Assist patient with hygiene while educating and training caregivers and loved ones on assisting with bathing and self-care.
Review physician orders, medical histories, and diagnoses
Identify functional deficits and establish patient-centered goals
Develop individualized treatment plans tailored to each patient's functional needs
Implement therapeutic interventions to improve strength, coordination, balance, and independence
Train patients in adaptive techniques to improve daily living skills
Recommend and train patients on the use of adaptive equipment or assistive devices
Educate patients and caregivers on safe mobility, home safety, and fall prevention
Provide instruction on energy conservation techniques and body mechanics
Develop personalized home exercise and functional training programs
Monitor and document patient progress toward therapy goals
Adjust treatment plans based on patient response and outcomes
Communicate regularly with physicians and the interdisciplinary care team
Work closely with nurses, physical therapists, physicians, and case managers
Participate in care coordination to ensure comprehensive patient care
Provide recommendations for continued therapy or discharge planning
Maintain accurate and timely patient documentation
Complete all required records for billing, compliance, and insurance
Ensure compliance with HIPAA regulations and agency policies

✔ Active Arizona Occupational Therapist (OT) License
✔ Home health experience preferred but not required
✔ Strong clinical assessment and problem-solving skills
✔ Excellent communication and patient education abilities
✔ Ability to work independently and manage a field caseload
✔ Passion for helping patients regain independence and quality of life
