Job Summary :
- The Behavioral Health Specialist is responsible for providing behavioral health services to participants identified by the Interdisciplinary Team (IDT) as needing behavioral health assessment, counseling, crisis intervention, or other therapeutic interventions.
- The Behavioral Health Specialist will deliver services to participants at the Center, inpatient setting or in the homes and collaborate with the PACE physician and consulting psychiatrist, as necessary.
- In addition, the Behavioral Health Specialist provides training to staff on behavioral health symptoms and appropriate interventions to support the dignity and highest level of functioning of participants with behavioral health issues.
- Collaborates effectively with colleagues and stakeholders to promote the values, team culture and mission.
Duties and Responsibilities :
- Conduct behavioral health assessments to determine the behavioral health care needs, preferences and goals of the participants and actively participate in IDT meetings to develop discipline-specific goals and interventions.
- Deliver and document behavioral health interventions as agreed upon in the participants’ care plans including but not limited to symptom management, counseling, crisis intervention, arranging necessary resources and services, assisting with care transitions, providing individual and group counseling and care management.
- Facilitate individual and group therapies in various forms as appropriate such as art and music therapy, psychosocial drama, counseling and discussion that restore or expand participants’ social roles.
- Provide training on behavioral health symptoms and interventions to staff.
- Provide mentorship and clinical supervision to Social Workers (MSMs) in their training to become Licensed Clinical Social Workers, as assigned.
- Support the intake and enrollment process to ensure prospective participants move through the enrollment process in a timely and effective manner, including joining the intake team to screen prospective enrollees as requested.
- Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants’ condition or medical status.
- Be legally authorized to practice in position location, required.
- Only act within the scope of his or her authority to practice.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated trainings and education.
- Timely completion of all mandated occupational health screenings as needed.
- Exercises flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Required Skills & Experience :
- Minimum of five years proving direct services to people with chronic mental illness and substance abuse issues.
- Minimum of two years of experience with a frail or elderly population.
- Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families, and interdisciplinary team members.
- Experience coordinating and facilitating care conference meetings.
- Ability to facilitate problem-solving with challenging psychosocial and family situations.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.
- Commitment to unlocking the full potential of our most vulnerable seniors.
Preferred Skills & Experience :
- Prior experience coaching others in a supervisory capacity.
- Familiarity with local community agencies and resources.
- Bilingual English / Spanish.
