Are you seeking a rewarding and challenging opportunity as a Hospice LPN ? We are looking for dedicated, compassionate, and organized individuals with hospice experience to join our Gilbert Team and become a valued part of our Hospice Family .
In this role, you’ll be providing both clinical care and emotional support—helping patients live their final days with dignity, comfort, and respect. You’ll also :
- Conduct supervisory visits to assess performance of care team members
- Collaborate in interdisciplinary group meetings
- Participate in the development, review, and revision of individualized care plans for patients and their families
To be successful in this role, you must be a licensed LPN with a strong belief in the value and importance of hospice care.

Requirements
Current Arizona License in good standingEmpathetic approach to patients and caregiversParticipate in field visits daily, weekly office meetings as indicatedAbility to adapt to patient’s status and needs on a full time on call basisJob Responsibilities :
Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needsImplement the individualized plan of care and revisions as neededCompliance with AdvisaCare Hospice Policies and Procedures, Medicare complianceConsulting with and educating the patient / family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concernSupervision of paraprofessionals providing services to patientPreparing clinical and progress notes, participating in IDG conferencesParticipate in Hospice performance improvement programMaintain high level of customer satisfaction with our clients, families and caregiversCommunication of information using current process and technology within organizationBenefits
