What you’ll do
You will be responsible for the provision of preventative medial, palliative, and educational services to promote patient health and wellness.
You will also conduct comprehensive medical and social histories and perform physical examinations on healthy, acutely ill, chronically ill patients or terminal patients.
What it takes
- Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP) required
- Current Certificate of Prescriptive Authority or Externship Certificate to Prescribe
- Two years’ experience in the home health and / or hospice field required
- Current and valid driver’s license as well as acceptable driving record required
