The Speech Language Pathologist partners with residents and other care providers to provide Speech Language Pathology services to maximize their function and independence in their daily activities.
Essential Duties and Responsibilities
Supports and promotes the mission and values of Covenant Health Ministry.
Performs evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider while observing precautions, indications, and contraindications.
Provides evaluation and treatment in the areas of communication disorders and dysphagia, including modified barium swallow.
Evaluate residents per department protocols and establish a therapeutic plan of care that includes functional long- and short-term goals. Frequency and duration of treatment are specified in addition to rehabilitation potential.
Seeks input from the residents and primary caregivers regarding the therapeutic plan of care.
Provides consultation to the referring physician and other members of the healthcare team regarding the therapeutic and discharge needs of the resident. Attends all pertinent team meetings.
Monitors and evaluates the treatment effect and progresses the treatment program toward the functional goals established in the treatment plan of care. Modifies the treatment plan of care as necessary.
Documents all evaluative and therapeutic interactions with the resident per department procedures. Ensures that the documentation is accessible to others on the care team using language that can be easily understood.
Recommends and obtains appropriate communication / swallowing assistive devices to the resident ensuring that instruction has been given in their use.
Provide appropriate education to the resident or caregiver. Documents response to teaching.
Understand Medicare, Medicaid, and other reimbursement regulations.
Maintains clinical knowledge, skills, and competence and shares expertise with others.
Provides in-services regarding clinical knowledge and skills to nursing and other departments.
Actively assists with the development of new programs and services in Speech Therapy.
Maintains clinical skills and competence by awareness of professional updates through membership in professional organizations and reading professional publications. Bring pertinent issues to the attention of others on staff.
Maintains required licenses, certifications, and competencies, and completes annual compliance courses on time.
Coordinates and ensures Speech and Language coverage.
Ensures department cleanliness and maintenance by routinely keeping office and clinical areas clean and neat. Assists with routine equipment and space cleaning. Follows all system and department infection control procedures.

Education and Experience
Masters degree required
Speech & Language Pathologist license required
Certificate of Clinical Competence required
Basic Life Support (BLS) certification preferred
Prior experience in a long-term care or skilled nursing facility preferred
Working Conditions / Physical Demands
Must possess the physical and mental abilities to perform the tasks normally associated with this position that involve a combination of sitting, standing, walking, bending, stooping, twisting, and reaching.
Must be able to lift up to 50 pounds. Above 50 pounds requires a 2 person lift.
Lifting / carrying frequently up to 20 pounds. Some stress is related to a high level of responsibility for quality care.
