• Minimum 2 years of recent Case Management experience in acute care settings\n
• Proficiency with EMR systems, including Meditech and Careport\n\nCase Management RN Responsibilities\n
• Coordinate and manage patient care for a designated population within the acute care facility\n
• Work collaboratively with interdisciplinary healthcare teams to develop and implement individualized care plans\n
• Assess patient needs using established standards, Care Maps, and clinical guidelines\n
• Communicate and coordinate with third-party representatives to optimize patient care and reimbursement\n
