Social Worker
HealthCare Support is actively seeking Social Worker to fill an opening with a long term care organization in Ridgewood, NY .
Daily Responsibilities for Social Worker :
- Provides social work consultation to members and authorized representatives as indicated and requested.
- Provide education on treatment options and help coordinate services.
- The Social Worker is responsible for the completion of psychosocial, mental status tools assessment, MOLST, GDS (when applicable), and other tools as appropriate,
- Completing comprehensive assessments via the McKesson, Care Compass, Epaces, and other systems as needed.
- Participates in peer supervision conferences and individual conferences with the Social Work Manager / designee.
- Represents the Social Work Department and Company at appropriate meeting and conferences in the community.
- All other duties as assigned by Supervisor.
Schedule : 9 : 00am - 5 : 00pm - 1 HR break and two 15-minute breaks if needed
Required Qualifications for Social Worker :
Education : M.S.W. from an accredited college or university
Experience : One year experience in a health care setting. Must have a car for field visits
License : Current certificate and licensure from New York State
A Vehicle is needed for the role, experience with Geriatric Population, and Field work required

Pay Details : $40.00 - $45.00 an hour
