Now seeking a Licensed Clinical Social Worker Case Manager for a full-time position at Luke Air Force Base in Glendale, AZ.
Core Duties :
- Participate in Utilization Management / Case Management meetings to review and identify opportunities to improve treatment provided and recommend corrective action when problems exist.
- Participate in the provision of monthly in-service training for non-healthcare-practitioner members of the clinical and administrative staff on subjects germane to social work and social work CM services.
- Provide patient referrals.
- Interface with the facility's RN Case Manager(s) in the development and implementation of the Case Management Program (CMP).
- Develop local strategies that use the strength and disciplines of both nursing and social work to accomplish case management.
- Provide assistance in the development and implementation of educational programs.
- Additionally, the social work case manager may be required to provide those services identified under Licensed Clinical Social Worker.
- Coordinate appropriate referrals to community and other Government agencies for services.
- Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient’s problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
- Independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.
- Use medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses / injuries, common medications and their effects / side effects, and medical terminology.
- Implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals.
- Independently provide counseling and / or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification.
- Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
- Participate in professional peer review case conferences, research studies, or other organizations required at the MTF.
- Must not have any suspensions of clinical licensure.
Type of work includes the following :
Responsible for and apply social work procedures and techniques in the evaluation and treatment of psychological / psychiatric disorders. Activities include, but are not limited to, the following :Responsible for and provide adult individual, AD only in most cases family and group psychotherapy.Consult to medical personnel, legal authorities, military commanders and school districts, as required.Maintain accurate and current notes in both the Mental Health records and patient medical records of all patients seen, as appropriate, and produce reports of evaluation and / or treatment, as required.Treatment. Provide treatment across a range of modalities to include Psychotherapy for AD population.Consultation. Consult with other individuals and organizations to include other hospital services (e.g., Pediatrics, Neurology, Social Work), public schools, courts, the Exceptional Family Member Program and social services agencies regarding specific case management and programmatic issues affecting children and families. Provide consultation and feedback to parents in order to relay results of treatment. Collaborate with other professionals in order to provide social work services to children, and in making interventions within the child's psychosocial environment. Provide consultation which includes not only traditional clinical interventions that emphasize delimiting and remediating psychopathology , but prevention, and the encouragement of psychological health.Research. Participate in and / or direct clinical research and conducts clinical investigations in socialwork.
Independently provide counseling and / or psychotherapy services to individuals, groups. Conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.Provide counseling and / or psychotherapy services to individuals, groups, and families.Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.Evaluate own practice through participation in a professional peer review case conferences or other organized means.Must possess the knowledge and experience to :Use computer software applications for drafting documents, data management, and tracking, especially those programs in use by DoD.Use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illness / injuries, common medications and their effects / side effects, and medical terminology.Implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals.Demonstrate a working knowledge of professional standards and ethics regarding the delivery of clinical social work services.
Work Schedule :
Monday - Friday, 9 hours between 7 : 00am and 5 : 00pm, including a one hour lunch
No Weekends, No Holidays
Requirements
Minimum Qualifications :
Education : Master’s degree in Social Work (MSW). Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
Experience : Must have at least minimum of one (1) year in the independent practice of clinical social work / healthcare management for adults, children, families, seniors, or groups within the past five (5) years.
License : Active, unrestricted license to practice as a Licensed Clinical Social Worker from any state. Please identify state of licensure.
Life Support Certification : Current AHA or ARC BLS Healthcare Provider certification.
Security : Must possess ability to pass a Government background check / security clearance.
