- Conduct thorough assessments and identify patient-specific needs using age and area-appropriate tools.\n
- Develop and update individualized care plans in collaboration with patients, families, and interdisciplinary team members.\n
- Provide direct patient care aligned with nursing protocols and individual patient needs.\n
- Utilize best practices for quality improvement and patient safety, continuously monitoring the effectiveness of care delivered.\n
- Facilitate patient education and ensure a smooth transition of care post-discharge.\n
- Stay updated on regulatory standards and maintain compliance with all organizational policies.\n
- Participate in performance improvement initiatives to enhance patient outcomes.\n

- Education : Associate degree in Nursing; Bachelor's degree preferred.\n
