All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope.

Position: Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Requirements:

• Valid California Physical Therapy license.
• Current CPR certification.
• Valid California driver's license.
• Current California auto insurance.
• Completed physical examination, TB test, and required vaccines/titers.
• Two professional references.
