• Our Full Time Patient Care Coordinator (PCC) will be part of our agile team, making the future of complex chronic care delivery the way it should be – human-centered and personalized .
• Our PCC is the heartbeat of the support team.
• In this key role you’ll showcase your passion for administrative, hospitality, customer service, and hands on clinical skills; all in support of patient care.
• You will report to the Senior Director of Operations. This role will be on-site and an current Medical Assistant certificate is preferred.

What you’ll do:

• Assist with vital collection and documentation; as well as other general clinical assistance Deliver a positive, warm greeting to each patient as they walk in the clinic Dialogue via phone, email, text (via platform), and inquiries (general) from referring providers and patients in a timely and accurate manner Receive packages, order supplies, re/stock supplies, light housekeeping, and general clinic support functions Own suite and supply sanitation between patients What you’ll need: You will have 2+ years of clinical experience Active CPR Certification (or willing to get certification within 90 days of hire) Familiarity with injection therapy (infusion therapy good, but not required) Strong understanding of clinical processes and protocols Licensed Medical Assistant (MA) highly preferred What you’ll bring to the table: Ability to navigate and chart in Electronic Health Record (EHR) systems Ability to analyze, order, and synchronize multiple high impact priorities in a fast-paced work environment A positive, upbeat team player Strong customer service skills and an empathetic disposition Who you are: Optimizes Work Processes.
