This role requires a skilled and compassionate Licensed Vocational Nurse to provide high-quality in-home nursing care to patients. Working under the direction of a home health Registered Nurse, the successful candidate will utilize an established plan of care to deliver exceptional patient outcomes.
Responsibilities include working collaboratively with all disciplines as a team to provide quality patient care. This involves collaborating with physicians, family members, and other healthcare professionals to ensure seamless patient transitions and optimal health outcomes.
Required Skills and Qualifications :
- High School Diploma
- Associates Degree in Nursing (Required)
- 1 Year of experience as a Licensed Vocational Nurse (Required)
- Home Health experience (Preferred)
