At Houston Methodist the Physical Therapist(PT) position is responsible for assessing patients and developingthe therapy plan of care.

This position provides education topatients patients family members and / or caregivers and collaborateswith other health care professionals as needed for problemresolution and / or achievement of identified patient specific goals.

As a member of the patient care team the PT position providesdirect patient care and has responsibility to deliver exceptionalpatient care with adherence to safety standards and guidelines toensure optimal patient safety customer service and optimum patientoutcomes.

Seeks guidance from others and collaborateswith all members of the interprofessional care team by activelycommunicating and reporting pertinent patient care information anddata in a comprehensivemanner.

SERVICEESSENTIAL FUNCTIONS

Performscomplete and comprehensive evaluation of patients to identifypatients level of function and to determine the treatment plan.

Develops the plan of care based on evaluation findings andimplements the physical therapy treatment plan. Practices in amanner that is congruent with cultural diversity and inclusionprinciples.

  • Provideseducation to patients patients family members and / or caregiversbased on learning needs. Consistently evaluates the patientscomprehension and adapts teaching methodsaccordingly.
  • Collaboratesand communicates with the healthcare team modifications to patientstreatment plan progress and discharge planning for achievement ofidentified patient specific goals.

Advocates on patient / familysbehalf to identify and resolve clinical and ethical concernsutilizing appropriateresources.

Contributestowards improvement of department score for patient satisfactionthrough peertopeer accountability to servicestandards.

QUALITY / SAFETYESSENTIAL FUNCTIONS

  • Assessespatients regularly monitors progress towards treatment goalsmodifies goals and plan or program as indicated. Recognizescomplications / limitations associated with certain procedures andcollaborates with peers to addresses for optimumoutcomes.
  • Provides inputof physical therapyrelated activities completed byinterprofessional health care team and if necessary adjusts theplan to improve quality of outcome and maintain the safety of thepatient.
  • Identifiesopportunities for corrective action and process improvement.Identifies areas of practice improvements with researchbasedevidence to achieve patient safety outcomes supported by accuratedocumentation.

Contributes to meeting department and hospitaltargets for quality andsafety.

FINANCE ESSENTIALFUNCTIONS

Maintains accurateand complete documentation of patient education proceduresperformed in a timely manner in the patient record.

Enters accuratecharges for services provided as applicable.

Uses resourcesefficiently to maximize utilization of time and equipment.Selfmotivated to independently manage time effectively minimizingincidental overtime and prioritize daily tasks assisting coworkersas needed.

Meets labor productivity standards by effectivelymanaging established patient loadassignments.

GROWTH / INNOVATIONESSENTIAL FUNCTIONS

  • Offers innovativesolutions through evidencebased practice / performance improvementprojects. Actively participates in departmental projects andhospitalcommittees.
  • Identifies andpresents areas for innovation efficiency and improvement in patientcare or unit operations using evidencebased practiceliterature.
  • Seeksopportunities to expand learning beyond baseline competencies witha focus on continual development as a Physical Therapist.

Assistshigher level PTs with program development and / or clinicalcompetencies. Ensures own career discussions occur with appropriatemanagement.

Completes and updates the My Development Plan on anongoing basis.
