As the Field Operations Specialist you will be responsible for conducting service coordination and home visits for the care management team.  You will be r esponsible for the clerical, administrative, customer service and coordination of activities to support care management.

Key Responsibilities :

• Travels to members’ homes and conducts in-home care management visits
• Communicates effectively with members / caregivers and Plan Team (clinical and non-clinical)
• Accurately documentations and uses the electronic medical record
• Schedules care management visits timely and effectively.
• Submits timely and accurately for   travel reimbursement expenses
• Reads reports timely and manages the queue to schedule care management visits in a timely manner
