Licensed Care Manager

A company is looking for a Care Manager to collaborate with an inter-disciplinary care team to improve member health outcomes through coordinated care and support.

Key Responsibilities

Engage members and their support systems through assessments and care planning

Coordinate care and facilitate communication among the inter-disciplinary care team

Monitor member progress and satisfaction while managing care plan goals and barriers

Required Qualifications and Education

Nursing degree from an accredited program or Bachelor's degree in a healthcare field, or equivalent experience

Advanced degree associated with clinical licensure is preferred

Minimum of three (3) years of experience in nursing, social work, or healthcare professions

Preferred experience in Medicaid and / or Medicare managed care

Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor is required
