Job Responsibilities

• Perform comprehensive patient evaluations and assessments to identify physical therapy needs and functional limitations
• Design and implement individualized treatment plans focused on improving patient mobility, strength, balance, and overall physical function
• Deliver evidence-based therapeutic interventions, manual therapy techniques, and rehabilitative exercises tailored to each patient's goals
• Provide patient and caregiver education on home exercise programs, adaptive equipment, and fall prevention strategies
• Collaborate with physicians, nurses, occupational therapists, and other interdisciplinary team members to ensure coordinated, comprehensive care
• Maintain accurate and timely clinical documentation in compliance with Medicare, Medicaid, and insurance requirements
• Monitor patient progress regularly and adjust treatment protocols to maximize functional outcomes
• Conduct home safety assessments and recommend environmental modifications as needed
