Job Title : Managed Care Community Connector

Location : Remote + Field Visits

Duration : 6 Months+ (possible to extend or TEMP TO HIRE)

Shift : Monday to Friday 8 : 30 am to 5 : 00 pm EST

Pay Rate : $27- $27.91 / hr. on W2

HIRING 2 TEMPS IN THE NYC –

1 TEMP IN SYRACUSE AND 1 IN NYC AREA FOR WESTCHESTER AND LONG ISLAND.

Job Description : Must Have Skills :

• Strong verbal and written communication skills, with the ability to engage compassionately and clearly with members.
• Active listening and empathy to build trust with vulnerable and underserved populations.
• Organizational and time management skills to schedule, conduct and document multiple home visits.
• Ability to work independently in the field while collaborating effectively with Care Management team.
• Cultural Competency and sensitivity when working with diverse backgrounds and community needs.
• Basic proficiency with mobile devices and Microsoft platforms.
• Bilingual is preferred (Preferred languages : Bengali, Russian, Chinese, or Spanish)
